Dr. Meynen closed her practice and retired effective 1 July 2020.

Requests for medical records must be sent via USPS mail to:

The request must include:

• The name of the requesting entity or person, a direct email address, and a direct telephone number
• The purpose for the request
• The USPS address to which a reply will be sent
• A signed statement from the patient requesting the records be sent as requested
• The patient's name, date of last visit (if known), and telephone number

Dr. Meynen may contact the requesting entity for clarification. The record provided will be a medical summary of the patient's condition and notable issues as of the last office visit or July 2020, whichever is earlier.

If the request is for a full copy of the patient's medical file, Dr. Meynen will contact the requesting entity with the expected fee for duplication.
